Apply For Passport Services and document(s) required

Applicant is required to fill up online at https://portal3.passportindia. After filling in the details and successfully acceptance of the application online, a Web File application form is generated that can be printed by the applicant. This printed copy becomes the application form.

Please note that “Tatkal Passport Service” is not available with the Embassy of India, Ljubljana. The applicant needs to select normal passport service while filling up the application form.

1) Issue / Re-issue of Passports

1. Print-out passport application needs to be signed on two designated places (on first page under the photograph and the last page of application form). All signatures in the application form should be signed with ballpoint pen of blue ink.

2.  Those who cannot affix their signature like infants and minors they should put their thumb impression instead of signature below the photograph. Thumb impression should be of left hand in case of males and right hand in case of females. In case of minors, either of the parent needs to affix his/her signature on the last page of the applicant form.

3. It is mandatory that the signature/thumb impression of the applicant should fit exactly in the prescribed signature box provided on the first page as it is scanned and printed on the passport. Unsigned applications will be rejected.

2) Issuance of First Passport to a New Born Child

It involves two steps:

a. FIRST STEP (REGISTRATION OF BIRTH/BIRTH CERTIFICATE)

(i) The registration of birth of the child be registered on Ministry of Home Affairs (MHA) to the Government of India’s website (Please access the link below):

Section 4 - Acquisition of Indian citizenship by descent [Registration of birth of a minor child at an Indian Consulate Under section 4(1) of the Citizenship Act, 1955]

Note: To register a child born in Slovenia as an Indian citizen, the application must be submitted within one year of the child’s birth using the prescribed form. At least one parent must be an Indian citizen at the time of application.

bSECOND STEP (APPLICATION FOR PASSPORT)

Following procedure may be followed for applying for fresh passport to a new born child:

(i) Print-out of the passport application form filled online be taken. Instead of signature, thumb impression of the new born child is required in signature box on the first page and either parent's signature on last page of the application form (left hand thumb impression in case of male child and right hand thumb impression in case of female child)

(ii) Copy of Birth Certificate issued by the local Slovenian Authority

(iii) 02 recent colour photographs with white background (5 cm x 5 cm) (one pasted on the designated place in the application form and other stapled on the last page of application form)

(iv) Self-attested copies of parents' passports and Slovenian Residence Permits / Visas

(v) Proof of payment for the applications (Issuance of birth certificate + Passport for the child)

Note : The application fee must be deposited directly into the Embassy’s account. Once the payment is made, the completed application must be submitted to the Embassy within 15 days. Please ensure that a copy of the transaction slip is attached to the application as proof of payment.

Application for birth registration and fresh passport may be submitted together. Passport application of new born child without Birth Registration Certificate /application for Birth Registration Certificate would be rejected.

Submission of Application Forms:

The Embassy accepts completed applications at Embassy located on the 1st Floor, Železna cesta 16, 1000 Ljubljana, Slovenia, between 10:00 and 12:00 on Wednesdays and Fridays. No prior appointment is required.

CHECK LIST OF DOCUMENTS FOR DIFFERENT CATEGOIRES

(If there is any change in the particulars of the applicant from the existing passport, copy of supporting document is mandatory, otherwise the applications will be rejected )

1. Re-issue of passport on completion of validity of current passport/exhaustion of pages:

(i) Passport Application Form as mentioned above

(ii) 02 recent photographs - 5 cm x 5 cm (one pasted on the designated place in the application form and another attach with the application form) - For Photo Specification: https://ociservices.gov.in/Photo-Spec-FINAL.pdf

(iii) Original Passport and one copy of front and last page of the Passport. (Original Passport is mandatory, otherwise application will be rejected)

(iv) Copy of Slovenian Resident Card/Visa

2. Passport for children below 18 years of age:

In addition to the documents mentioned at Para (1) above (Re-issue of passport), following is also required :

(i) Consent letter (Link) from parents

Note: A minor’s passport is valid for five years or until they turn 18, whichever comes first. However, minors aged 15 to 18 have the option to apply for a 10-year validity passport by paying fees for normal passport.

4. Re-issue of passport for splitting name (writing name and surname in different columns in the passport):

In addition to the documents mentioned at Para (1) above (Re-issue of passport), following is also required :

(i) Affidavit for name split

5. Re-issue of passport for inclusion of spouse name in the passport:

In addition to the documents mentioned at Para (1) above (Re-issue of passport), following is also required :

(i) Marriage Certificate (apostilled by the Ministry of External Affairs, India), if issued by Slovenian Government, it should be apostilled by either Ministry of External Affairs or Ministry of Justice, Slovenia

6. Re-issue of passport in lieu of lost or damaged passport

In addition to the documents mentioned at Para (1) above (Re-issue of passport), following is also required :

(i) Photocopy of the lost passport (first page and last page). Passport in original in case of damaged passport

(ii) Police report, in original, for the lost passport containing the lost passport number

(iii) An Affidavit [Link] regarding loss of passport/damaged passport.

(In case of frequent loss/damage of passport, additional penalty will be separately charged and decided on a case to case basis).

7. Re-issue of passport due to change of name after marriage or remarriage

In addition to the documents mentioned at Para (1) above (Re-issue of passport), following is also required :

(i)Passport Copy of spouse (first page and last page)

(ii) Marriage Certificate (Marriage Certificate (apostilled by the Ministry of External Affairs, India), if issued by Slovenian Government, it should be apostilled by either Ministry of External Affairs or Ministry of Justice, Slovenia 

(iii) Attested copy of divorce decree (For name change after divorce)

8. Re-issue of passport due to change of name after divorce/death of spouse

In addition to the documents mentioned at Para (1) above (Re-issue of passport), following is also required :

(i) Passport Copy of spouse (first page and last page)

  • In case of Divorce: Copy of Decree of Divorce attested by CPV Division, Ministry of External Affairs, New Delhi. Original may be presented to Consular Officer if asked to produce the same. If the Decree of Divorce is issued by local court in Slovenia, the same should be notarized and attested by the Slovenian court.

  • In the case of Death of Spouse: Copy of Death Certificate attested by CPV Division, Ministry of External Affairs, New Delhi. Original may be presented to Consular Officer if asked to produce the same.

9. Re-issue of passport due to change in name (For reasons other than marriage or divorce)

In addition to the documents mentioned at Para (1) above (Re-issue of passport), following is also required :

(i) Clippings of two leading Indian newspapers or the Gazette notification of the concerned state government in regard to change of name.

(ii) Any two public documents (school certificate/ Aadhar card/ Voter id card etc. ) issued in the desired name.
